PLEASE NOTE: WestCOP requires the following information :
- Verification of Identity (photo identification from the Head of Household and all Household Member Over the Age of 18. Birth certificates for minors
- Verification of Address – Current address on Photo ID or any correspondence from a Federal, State, County or City Agency
- Proof of Income from the Head of Household and Household Members Over the Age of 18
- Pay Stubs from the last 30 days, Social Security Award Letters, Pension Letters, etc.
- Social Security Numbers are required for certain Programs and Services
- Other Supporting Documentation may be required for certain Programs and Services
